Francophone Health System Navigator

Lire en français

The Francophone Health System Navigator (FHSN) is a pilot project that seeks to provide an additional level of support to French-speaking patients and families to improve their navigation and access through the health-care system.

Patient navigators are part of a patient’s support system and offer guidance through the health-care system. Navigators link patients to available resources, information and people best suited to help them improve their health and quality of life.

Who is the Francophone Health System Navigator?

The Francophone Health System Navigator is a registered nurse who works closely with patients, families and other clinicians and community partners to make sure that French-speaking patients have the information and knowledge they need to make the best decisions about their care.

What is the role of the Francophone Health System Navigator?

The Francophone Health System Navigator helps French-speaking individuals and families find the health services they need. The role of the FHSN is to:

  • Facilitate access to required health-care services for Francophone clients and their families.

  • Arrange non-urgent interpretation services (oral) for francophone clients.
  • Liaise with available internal and external resources in French and coordinate services for individuals and families.
  • Facilitate education and information-sharing about health services in French.
