How to request your health records
Requests for your personal health records can be made by filling out the Consent for Release/Request of Personal Health Information form (PDF) and sending it:

The age of consent for authorizing access to information is 16 years. If the requested health records are for an individual between 12 to 15 years, the signatures of both the patient and the parent or legal guardian are required.
If you are requesting the medical records of someone other than yourself, you require the written and explicit consent of that person. If the person whose medical records you are requesting has passed away or is otherwise unable to provide written and explicit consent, please consult with Eastern Health for further instructions by contacting us by telephone at 709-777-8025 or via email at firstname.lastname@example.org.
Due to the COVID-19 pandemic, counter service is not available and in-person requests are not accepted at this time. References to in-person service and the limitation on emailing and faxing of medical records in the Consent for Release/Request of Personal Health Information form do not apply currently.
How long will it take to process my application?
We work to process your request as quickly as possible; however, processing times vary. The length of time it takes to process a request depends on the type of information being requested and the number of other requests currently being processed.
Please submit your request in a timely manner, particularly if you require the information for income tax; estate planning; or time sensitive, decision-making purposes.
Is there a cost for processing my request?
Fees will be charged in accordance with our Release of Information Fee Schedule. In some situations, a deposit will be required prior to processing your request.
