Attending an appointment at Eastern Health

Patients should arrive no more than 15 minutes prior to their scheduled appointment at any Eastern Health facility.

All patients who receive health-care services by Eastern Health must be registered before attending their appointment to ensure the accuracy, privacy, and integrity of the patient health record.

Patients are encouraged to maintain a current Medical Care Plan (MCP) card and to complete necessary forms for new cards prior to attending appointments at Eastern Health.

In-person registration

In person registration is available for all appointments at all Eastern Health facilities.

Appointment registration can be completed upon entry to the Eastern Health facility at which the appointment has been scheduled.

Documents required to register for an appointment

  • Valid health insurance card (MCP or provincial health card)
  • Doctor referral or requisition if one has been provided for the appointment

If a patient’s MCP has expired within the last 12 months, they will be asked on site to complete a form to request a new MCP Card and their MCP will be recorded for the appointment.

If the patient’s MCP has expired more than twelve months, the patient will be asked to complete a form to request a new MCP Card; however, it will result in an invoice sent from Eastern Health for the appointment.

Documents required if patient is from another province:

  • A valid provincial health card.
  • Doctor referral or requisition if one has been provided for the appointment.

If a patient presents an out-of-province health card, a photocopy of the card will be made by the registration clerk so that billing may occur to that health care authority.

Documents required if patient is not a Canadian citizen (including St. Pierre residents):

  • Doctor referral or requisition if one has been provided for the appointment.
  • Immigration forms.
  • If patient is a St. Pierre resident, a social security fund document is required.

For patients residing outside of Canada, a Foreign Residents Agreement will be completed during registration.

If the appointment is related to a workers compensation claim:

  • Patients should know the workers compensation claim number and be able to identify which part of the body was injured in relation to the claim as well as the date of the injury.

Information required to register for an appointment:

To confirm positive client identification, the following information will be collected during each appointment registration:

  • Full legal name
  • Date of birth
  • MCP or out-of-province health card number
  • Mother’s name

The following information will be confirmed during each visit registration to identify and update any changes since the last visit:

  • MCP or out-of-province health card number expiry
  • Home address
  • Phone number
  • Next of kin
  • Person to notify
  • Attending physician
  • Family physician
  • Appointment notification preference (phone, email or text)
  • Name, address and phone number of any additional specialists you are seeing such as cardiologists, nephrologists, respirologists.
