The Community care program is an initiative of Eastern Health’s Long Term Care Program. It is a community based supportive housing program for individuals who require support as a result of a severe and persistent mental illness.
The Community Care Program provides residential, psychiatric, case management and therapeutic recreation services to voluntary residents. Community Care Homes are privately owned and operated with the majority being located in Conception Bay area.
There are currently 12 Community Care Homes which are staffed 24 hours a day with a total of 163 placements in the program. Homes range in size from 6 to 22 resident placements. For the majority of residents who reside in the Community Care Program their goal is to secure a safe and supportive living environment, improve their overall quality of life and strive for longer periods of psychiatric stability. For a smaller number of residents their goal is to maintain wellness while focusing on the skills needed to progress towards a more independent lifestyle
