Janeway Referral Information

To make a referral to speech-language pathologist you must send a written referral including the child’s name, date of birth, MCP #, parents’ names, contact information (address and phone) and the reason for referral. Parents or guardians can refer their child for an assessment. A doctor’s referral is not required.

Once the referral is received, you will be sent a questionnaire to complete, which must be returned to us in order to complete the referral process.

*Note: Your child’s name will not be added to the waitlist until the questionnaire is received. Your completed questionnaire is then reviewed by the intake team to determine if other specific services may be required to meet your child’s needs. Your child’s name is then placed on the speech-language pathology wait-list for assessment.

Attendance Policy

If a new patient or re-evaluation appointment is not kept without prior notice of cancellation and notification is not received by this office within 48 hours following the missed appointment, the patient will be discharged from speech-language pathology waitlist.  If an appropriate re-referral is received, the child’s name will be placed on the S-LP waitlist as per normal re-referral protocol.

Therapy may be discontinued if:

  • A session is missed without prior notification and the office is not notified within 48 hours after the missed appointment, or
  • If three out of five consecutively scheduled appointments are cancelled
