Individual Award

Leadership

An individual who:

  • Demonstrates a willingness to engage others.
  • Demonstrates leadership skills and capabilities.
  • Promotes the diversity of people and ideas, work/life balance, recognition and teamwork practices.
  • Mentors, coaches, empowers, and supports employees in all phases of their careers.
  • Creates and sustains an effective and engaging work environment.
  • Contributes and supports to team harmony.

Mentoring

An individual who:
  • Advises, trains, or coaches other employees and/or has sought to guide employees to achieve their goals.
  • Demonstrates a willingness to engage other employees and has sought to guide employees to achieve their goals.

Individual or Team Award

Human Spirit

An individual or team who:

  • Inspires others by their demonstrated commitment and meaningful contribution to improving the lives of the individuals or populations they serve including patients, clients, residents and/or communities. 
  • Lives the values of Eastern Health on a daily basis and excels in their commitment to celebrate the human spirit.
  • Goes above and beyond the call of duty and inspires others by the impact of their acts of kindness.

Innovation

An individual or a team who:

  • Develops and/or implements innovative ideas that result in a measureable increase in quality, efficiency and productivity.
  • Creates an original idea or uniquely adapts an existing program, process or concept which resulted in a long-term benefit to patients, clients, residents and/or to Eastern Health.

Safety

An individual or a team who:

  • Demonstrates a commitment to safety at all times through his/her work practices, and by participating in training opportunities, education sessions, etc. 
  • Shows an understanding of safety practices within a specific workplace setting or on an organization-wide basis; achieves visibility regarding safety issues.

Service

An individual or a team who:

  • Models and inspires other employees in their dedication to excellence in their work.
  • Demonstrates outstanding dedication to their work while maintaining a high level of commitment to Eastern Health’s vision and values.

Team Award

Connectedness


The team award may include interdepartmental, special project or other types of teams. Teams should normally not exceed 10 to 12 members. Team consistently demonstrates a commitment to fostering Eastern Health’s vision and values
through their work in providing care and/or service delivery.

A team who:

  • Contributes significant amount of time, energy and talent in supporting cooperation between programs/departments that results in improved service delivery and/or outcomes for clients/patients/residents.
  • Understands the importance of working together for the greater organization.
  • Enhances the ability of programs and departments to learn from one another. 
  • Seeks and creates opportunities to collaborate with others.
  • Develops, manages and maintains partnerships with community stakeholders to build capacity and strengthen the ability of people, groups and communities to effectively address local health issues and concerns to help achieve our vision for Healthy People, Healthy Communities.
