We recognize that young people and their families are often involved with many different agencies and service providers. Service coordination can be a challenge at the best of times. Our role is to support those involved to communicate, review, coordinate and to take a goal-oriented and strengths based approach.

Our mission is to work with youth in collaboration with their families and service providers to enhance their life at home, in school and in the community.
What we do

  • Assisting with transitioning (e.g. custody,  residential treatment, acute hospitalization, etc.)
  • Intensive case management 
  • Clinical social work and child and youth care interventions

Our Team

  • Provincial youth corrections mental health coordinator
  • Regional mental health resource coordinators 
  • Child and youth care counsellors
  • Addictions counsellor 

Youth mental health coordinators provide services within the Eastern Health region and assist with connecting clients to appropriate programs outside the region during transitioning.

Child and youth care counsellors work directly with youth and their families to build strong, healthy relationships and assist with skill development through the use of daily life events.

The youth corrections mental health coordinator provides province-wide services to the broader youth corrections population with an emphasis on young offenders with mental health needs.

Who we see

  • Young people with complex service needs, between the ages of 12 and 18, and their families.
  • Parcipation in this program is completely voluntary. 
Common reasons for referrals are:
  • Youth and their families may need support to connect with services or may have services already in place that require further coordination. We work directly with our clients to address specific needs in their home and in the community.


How do you access this program?

Referrals to the CONNECT team can be made by completing the Mental Health and Addictions referral form (PDF) and faxing it to (709) 752-6863.
