How can you be active in your care?

There are a number of things that you and your family can do to assist your health care team in providing safer care.
  • Tell the doctor, nurse, social worker or other health care professional about how you are feeling or changes you may be experiencing.
  • Know what medications you take, the side effects and who to notify should you have any questions.
  • Learn about and participate in discussions about your care or service to ensure that it is correct for you.
  • Use walking aids when needed to help prevent falls at home and wear proper footwear when walking.
  • Practice good hand hygiene to help prevent the spread of infections (i.e. before eating and after coughing, sneezing or using the washroom).
  • Get the annual flu shot as recommended by your health care provider.

Ask us questions about:

  • Your medications.
  • Your plan of care and the best options for you.
  • Assistance with locating services that may benefit you.
  • If a family member can be with you.
  • The names and roles of persons providing your care.
  • Our hand hygiene practices.
