Overview and Conceptual Diagram

Working in Health Promoting Ways, Where We Live, Work, Learn and Play is a comprehensive plan designed to support the integration of health promotion into practice and to improve the health of the population served by Eastern Health. The plan outlines the context for health promotion and prevention, principles of practice and the priority areas for action for the period from 2012‐17. Action plans have been developed for each priority for action and will focus primarily on primary prevention.

A conceptual diagram, adapted from the Department of Health and Human Resources, Government of Tasmania, provides an overview of the components of the Health Promotion Strategic Plan 2012‐17. The outer circle of the diagram represents the Principles of Practice.

These principles have their foundation in a number of landmark health promotion documents such as the Ottawa Charter for Health Promotion.

The middle section of the diagram represents the Priority Areas for Action. These priorities were identified through a review of Eastern Health community needs assessments, health status reports, and informal consultation. In addition, a set of criteria were applied to determine the priority areas for action.

The centre of the diagram represents the vision of Eastern Health ‐ Healthy People, Healthy Communities. As an organization, Eastern Health is committed to working with individuals, families, communities and partners to achieve this vision.
