If held for observation or you come in for tests or procedures, it is important that you update your personal information with the Health Records/Nursing  Staff.  If any of the following demographic information has changed, please inform staff:

• Mother’s First Name
• Martial Status
• Mailing Address
• Next of Kin
• Work Place
• Phone Numbers
• Family Physician
• Religion
• Other Pertinent Information
• Any  allergies – food or drug , please notify staff.
