Planning for your discharge from the hospital begins soon after you are admitted. You and your healthcare team will decide when you are able to leave the hospital. If you have any questions about leaving, please talk to a member of your healthcare team.

Check-out

• On the day of discharge, it is important to leave the hospital before 11am.  This will allow the room to be prepared for the next patient. 
• Take your information and instructions about medication, care at home and any follow-up appointments.
• Take all your personal belongings, including any medications you may have brought with you to the hospital.
• Disconnect your phone and television.
• Go to the cashier’s office to pay your bill. 

Care at Home

• If you will need help at home and are unable to arrange for this help, ask your nurse for assistance as soon as possible in your hospital stay.
• Once home and you require help, contact your local health and community services for assistance. 
• There may come a time during your hospital stay when you health needs can be better met through palliative, rehabilitative or long-term care at another facility, or at another hospital closer to your home.    Your health care team will discuss this with you and prepare you for your move.
