For further information about personal health information, visit the Your Rights section Access to Information and Protection of Privacy Act (ATIPPA) The Access to Information and Protection of Privacy Act is a provincial law that provides clarity for individual citizens regarding their right to information, and introduces a personal privacy regime to limit the collection, use and disclosure of personal information held by government or any of its agencies. Approximately 460 public bodies are subject to ATIPPA, including government departments and agencies, school boards, public post-secondary institutions, health boards and municipalities. The Access to Information provisions were effective Monday, January 17, 2005, and Privacy Provisions were proclaimed on January 16, 2008. At Eastern Health, members of the public may request information in the custody or control of Eastern Health. The request for information must be accompanied by the completed form and the $5 application fee. If submitting your request via e-mail or fax, please note that your request cannot be acted upon until receipt of this fee.
