Although the file containing your personal health information belongs to Eastern Health, the information in that file belongs to you.
You have a right to access your health record.

This does not mean, however, that you may take, remove or change your health record. Why? The health record is created by health professionals and contains information relevant to the care and services you receive from Eastern Health. We have a legal obligation to ensure that your health record is securely kept for your current and future health events.You have a right to know why we collect your information, what we do with it, and who has access to it.
Our staff, physicians and health professional students work together as a team to provide you with services. This may mean that some of your information will be shared amongst your team to make sure you are getting the best possible service.
At Eastern Health we regularly contact our patients, clients and residents by telephone and in some cases we need to leave a message on an answering service or with a person who is answering the phone.
