Disclosure
At Eastern Health, we are committed to providing our clients, patients and residents with quality health care services.
In the event that an adverse event occurs in the delivery of that care, we have implemented a
QRM-030 Disclosure Policy (PDF)
to help guide us in how and when we share that information with the person or persons impacted, and potentially with the general public.
Occurrences that have reached the client, have caused harm, or have the potential to cause future harm must be disclosed to the client and/or substitute decision maker (SDM) in a supportive and timely fashion. Disclosure of a close call that does not pose a future health risk is discretionary. All public communications related to disclosure is managed through Corporate Communications department in consultation with the President and Chief Executive Officer (CEO).
If you have a concern about your care or a question about the disclosure of your health information, we want to hear from you. Please contact our
Client Relations Office.
