SAINT LUKE'S HOMES
Saint Luke’s Homes is located in the west end of the city of St. John's, the nursing home has 126 long term care beds, including one respite bed available to those residing in the community. It also has a Heritage Room and its own Chapel. Besides the nursing home, there are 54 independent-living cottages, and the Bishop John Meaden Manor Complex with 76 apartments. Saint Luke's is the only nursing home to offer an Adult Day Program and is committed to providing a continuum of care from independent living to nursing care. Philosophy The philosophy and purpose of Saint Luke’s Homes suggest both general and specific objectives appropriate for achieving the following:
Admission All applications for admission are processed through a Single Entry System, coordinated by Health and Community Services, St. John's region.An assessor from Health and Community Services will then visit the person requesting admission to a nursing home and complete an assessment for placement. Nursing home placement assessments for people who are currently hospitalized are completed by the Social Worker in the hospital. Upon medical discharge, these applicants may be referred to a transition bed at the Hoyles-Escasoni Complex transition unit or The Leonard Miller Centre, to await placement. This transfer referral happens if the applicant is not admitted to a nursing home directly from hospital.Applicants can request three nursing homes of choice for placement.All applications are processed through an Assessment and Placement Committee where acute care, community and nursing homes are represented. The applicant's level of care is determined and the applicant's name is appropriately placed on the waiting list.When a vacancy occurs in a nursing home, a social worker on site contacts a Placement Coordinator at Health and Community Services. Applications are then forwarded to that nursing home, where an interdisciplinary review is completed. The social worker then contacts the appropriate applicant or family to offer placement.Services St. Luke’s Homes offers the following services:
Mildred Boyce Cottage - Mildred Boyce Cottage Hostel For FamiliesThe Mildred Boyce Cottage provides affordable, temporary accommodations to family members normally residing outside the St. John’s area, during times of resident admission, family crisis, or for regular visits with loved ones in nursing homes.
The Cottage is available to families of residents of any of the six nursing homes within the St. John’s Nursing Homes Board and is located at Saint Luke’s Homes.
The Cottage accommodates a maximum of four guests. The cottage rate for two people is $38 per night.
