HOYLES-ESCASONI COMPLEX
The Hoyles-Escasoni Complex has a total of 377 long term care beds with respite. In the east end of St. John's, the Complex is the largest nursing home facility in the region. It also contains a new 15-bed Transition Unit where patients released from acute care hospitals may stay, prior to moving into a long term care facility. The Complex also has an Achievement Centre for various creative activities, and a Protective Care unit.
Admission
All applications for admission are processed through a Single Entry System, coordinated by Health and Community Services, St. John's region.
An assessor from Health and Community Services will then visit the person requesting admission to a nursing home and complete an assessment for placement. Nursing home placement assessments for people who are currently hospitalized are completed by the Social Worker in the hospital. Upon medical discharge, these applicants may be referred to a transition bed at the Hoyles-Escasoni Complex transition unit or The Leonard Miller Centre, to await placement. This transfer referral happens if the applicant is not admitted to a nursing home directly from hospital.
Applicants can request three nursing homes of choice for placement.
All applications are processed through an Assessment and Placement Committee where acute care, community and nursing homes are represented. The applicant's level of care is determined and the applicant's name is appropriately placed on the waiting list.
When a vacancy occurs in a nursing home, a social worker on site contacts a Placement Coordinator at Health and Community Services. Applications are then forwarded to that nursing home, where an interdisciplinary review is completed. The social worker then contacts the appropriate applicant or family to offer placement.
