Aboriginal Patient Navigator
The Aboriginal Patient Navigator works as an embedded member of a multidisciplinary team, supporting the provision of health care to Aboriginal patients/clients, caregivers and their families in a culturally- specific and ethno- sensitive manner.

Advance Care Planning
Advance care planning enables you to have a say about the health care you wish to receive if you get very sick and cannot speak for yourself.
Cancer Care Navigator
The Cancer Care Navigator offers practical help to bridge the gap and assist patients, families and caregivers to understand and work through the treatments, services, decisions and challenges encountered on their cancer journey.
Chronic Disease Prevention and Management
Many seniors deal with chronic diseases. Eastern Health has a coordinated, systematic approach to chronic disease prevention and management to positively impact the health of our people and communities.
Community Organizations
There are many community organizations that offer a range of health-related programs and services. Many of these complement Eastern Health’s programs and series or offer opportunities for partnerships.
Community Supports Program
The Community Supports Program assists individuals, families and care providers by promoting independence, community inclusion, safety and overall well-being.
Falls Prevention
Falls can happen to anyone, but as we age, falls can have a greater physical impact/we are more at risk. A serious fall can affect your quality of life. However, falls are preventable - and are not a natural part of aging.

Home and Community Care
Home and Community Care offers a range of at-home and community services to support people and their families who live in the community, but who need additional care and professional services.
Influenza
Seniors can be particularly vulnerable to the various strains of influenza that show up each year, usually in the fall and early winter. Learn more about this highly contagious respiratory disease, and our seasonal flu clinics.
Long-Term Care
Long-term care provides a range of residential and day services, which are mainly provided to seniors and older adults with disabilities. We support individuals’ choices, wellness and independence within our long-term care settings.
Mental Health and Addictions
The Mental Health and Addictions (MHA) Program offers a range of services. These services target people with mental health and substance use/gambling problems; addictions and mental illnesses.
Mental Health and Addictions Systems Navigator
Helps individuals and families to navigate large and complex health care organizations, as well as relevant government departments and community organizations.
Nursing Homes
Nursing Homes are publicly-funded facilities that offer care to people who need moderate to total assistance with daily functioning and daily on-site nursing care (three hours or more daily). There are also private pay facilities that offer higher levels of care (e.g. Chancellor Park).
Palliative Care
Eastern Health offers services to help relieve suffering and improve quality of life for people with a life-limiting illness. These services are provided within our acute care facilities, as well as in the home setting.
Palliative Care Navigator
Palliative care assists patients who are dealing with life-limiting conditions. The Palliative Care Nurse Navigator can be contacted at 709-777-7303 or toll free at 1-855-690-7303.
Personal Care Homes
Personal care homes are licensed, privately owned and operated residential homes that are overseen by Eastern Health in consultation with Service NL.
Remote Patient Monitoring (RMP)
Respite Care
Respite care allows a loved one to be placed in a long-term care facility for a short-stay.
Seniors Housing
This program provides affordable housing to older adults with disabilities and seniors on limited incomes.
